Main Purpose of Job

The Contractual Community Health Nurse II is the full performance level of direct nursing care to patients in various community settings and local health department clinics.  The Contractual Community Health Nurses will work in Montgomery County and complete the comprehensive medical assessment process using the standardized medical assessment, the interRAI, for applicants and participants of community-based long-term care programs including the Medical Assistance Personal Care program, Community First Choice and the newly merged Home and Community-Based Options Waiver (formerly Living at Home and Older Adults).  The assessment must be completed prior to entry into service and annually as it determines if an applicant meets the nursing facility level of care and the MAPC medical necessity criteria.  The incumbents will use the comprehensive assessment to generate an annual individual budget recommendation and create a recommended plan of care for participants.  The assessment tool and process are crucial to determining medical eligibility for programs and individual services within the programs.  These positions will require travel around Montgomery County going to recipients' homes to do medical assessments.

MINIMUM EDUCATION OR GENERAL REQUIREMENTS

Education:    A Bachelor’s degree in nursing from an accredited college or university.

Experience:  None.

NOTES:

1.  Candidates may substitute a Bachelor’s degree in a related field from an accredited college or university and one year of experience as a registered nurse for the required education.

2.  Candidates may substitute two years of experience as a registered nurse for the required education.

LICENSES, REGISTRATIONS AND CERTIFICATIONS

1.  Candidates for positions in this classification must possess a current license as a Registered Nurse from the Maryland State Board of Nursing, 4140 Patterson Avenue, Baltimore, Maryland 21215.

2.  Employees in this classification may be assigned duties which require the operation of a motor vehicle. Employees assigned such duties will be required to possess a motor vehicle operator’s license valid in the State of Maryland.

SPECIAL REQUIREMENTS

Employees in this classification will be required to provide the facility with a telephone number at which they can be reached.
