Main Purpose of Job
The main purpose of the Contractual Coordinator Special Programs, Health Services I is to provide Resource Coordination Services to eligible individuals with developmental disabilities who reside in Queen Anne's County. This Resource Coordinator determines eligibility for Developmental Disabilities Administration (DDA) services, provides referrals for services, accesses day/residential/support services, and facilitates the annual individual plan meetings, assists the individuals and their families to develop an annual individual plan, monitors individuals on the waiting list, and assists in transition planning. These efforts work to maintain the individuals in the community and to promote individual choice and their overall quality of life.
MINIMUM QUALIFICATIONS
Education: Possession of a bachelor’s degree from an accredited college or university in nursing, social work, psychology, education, counseling or a related field.
NOTE:
1. A bachelor’s degree from an accredited college or university in another field plus one year of professional experience in health services may be substituted for the specific degree.
2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.
