Main Purpose of Job

The Contractual Coordinator Special Programs, Health Services I is the entry level of work coordinating programs in the Department of Health and Mental Hygiene in a local health department, residential facility or the headquarters administration. The main purpose of this position for the Division of Addiction Services, Care Coordinators promote continuity of care and support patient efforts to access all levels of treatment, including residential detoxification. Care Coordinators also help individuals transition from secured and/or therapeutic residential facilities (including the county jail/detention center and regional residential substance abuse treatment programs) to community behavioral health services, half-way housing, recovery housing, and other recovery support programming. For the local jail, Care Coordinators enhance reentry services for offenders who express interest in Vivitrol medication, a prescription injectable medicine used to treat alcohol dependence and prevent relapse to opioid dependence after opioid detox.

MINIMUM QUALIFICATIONS

Education:  Possession of a bachelor’s degree from an accredited college or university in nursing, social work, psychology, education, counseling or a related field.

NOTE:

1.  A bachelor’s degree from an accredited college or university in another field plus one year of professional experience in health services may be substituted for the specific degree.

2.  Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.
