Main Purpose of Job

The purpose of this position is to work collaboratively and ensure continuity of communication with the Provider Relations Team, Quality Enhancement Team, Provider agencies and Community Support Services (CSS) Agencies to ensure people obtain and remain in the Home and Community Based Services (HBCS) waiver.  The job will assist CCS Agencies, families and providers in maintaining the waivered services for anyone in the Community Pathways Waiver.  Specific tasks and assignments may vary and change as new projects are developed or assigned to the region.

MINIMUM QUALIFICATIONS

Education: Possession of a bachelor’s degree from an accredited college or university in nursing, social work, psychology, education, counseling, or a related field.

Experience: Six years professional experience in health services, three of which must have been in professional work related to treatment and development of developmentally disabled clients, and two years of which must have been at the managerial or supervisory level.

Notes: 1. Applicants may substitute a master’s or Doctoral degree in a health or human services field for up to two years of the required general experience.

2. A bachelor’s degree from an accredited college or university in another field plus an additional year of professional experience in health services may be substituted for the specific degree.

3. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Health Services Administration specialty codes in the health related field of work on a year-for-year basis for the required experience and education.

Applicants who meet the minimum qualifications will be evaluated.  The evaluation may be a rating of your application based on your education, training and experience as they relate to the requirements of the position.  Therefore, it is essential that you provide complete and accurate information on your application.  Please report all related education, experience, dates and hours of work.  For education obtained outside the U.S., a copy of the equivalent American education as determined by a foreign credential evaluation service must accompany the application.  All information concerning your qualifications must be submitted by the closing date.  We will not consider information submitted after this date.  Applicants certified to list will receive eligibility for a period of one (1) year.  Eligibility may be extended beyond one (1) year period.
