Main Purpose of Job

Regional Clinical PharmD Pharmacist is to ensure the quality and accuracy of the daily workflow regarding pharmacy services, medication administrations and disease management, covering designated facilities or regions in the state assigned by the Chief Clinical PharmD. This position will also serve as a liaison between DPSCS and the medical and mental health clinical providers related to pharmacy issues, disease management, prescribing habits, medication audits and reports directly to the DPSCS Chief Clinical PharmD. This position will also monitor medication outcome measures by auditing, tracking and trending the drug utilization data submitted by the pharmacy vendor for the facilities associated with the assigned facilities. Conduct in-services related to disease and medication management as well as support the HIV and HCV, 340B process related to the region assigned. Perform medication room audits and provide coverage for the Chief Clinical Pharm D when requested and serve as a resource for the facility management staff regarding substance abuse and narcotic related overdoses. Monitor the offsite incidents related to medication misuse, errors, or complaints. Attend monthly regional Pharmacy and therapeutic regional committee chair meetings.

POSITION DUTIES

  • Provide clinical alternatives based on gold standard methodologies and cost effective formulary options.
  • Provide proper formulary management in order to keep the medication costs within budget resulting in the maximum number of patients to be treated, and monitors regional facility drug utilization. Review and approve/disapprove formulary exceptions.
  • Provide an assessment of quality assurance by reviewing, monitoring, compiling and evaluating data on medication errors to determine cause and an appropriate response.
  • Track medication consumption to determine if the utilization data is resulting in anomalies or inconsistencies for HIV and HCV medications and patients outcomes related to 340 B programs.
  • Work with the medical and mental health staff by providing drug information services which should assist in managing drug costs.
  • Provide direct communication with the prescribers and provide education which will lead to better prescribing habits which should be beneficial to the patients' overall health and wellbeing and be more cost effective.
  • Participate as a member of the work group (DPSCS Clinical Director, DPSCS Chief Clinical PharmD, Director of Nursing, and contractor staff) that assess quality indicators which could improve workflow and medication error prevention.
  • Submits reports as requested or required by the DPSCS Chief Clinical PharmD, Director of Clinical Services regarding all aspects of the DPSCS Pharmacy program, and the Executive Director of Field and Support Services
  • Attend Contractor and DPSCS meetings related to CQI, Infection Control, pharmacy

MINIMUM QUALIFICATIONS

Education: Graduation from an accredited School of Pharmacy with a doctorate in Clinical Pharmacy (PharmD). In order to meet this educational requirement a copy of your Clinical Pharmacy D degree must accompany your application.

For education obtained outside the United States, you will be required to provide proof of the equivalent American education as determined by a foreign credential evaluation service. This evaluation must be submitted with your application.

Experience: One year of clinical pharmacy experience as a Registered Pharmacist providing pharmacy services in a multidisciplinary community clinic, hospital facility,  or university.

DESIRED OR PREFERRED QUALIFICATIONS

Experience with Specialty Drugs, IV Drug Therapy and development and review of clinical criteria is highly preferred. Correctional pharmacy process experience preferred.

LICENSES, REGISTRATIONS AND CERTIFICATIONS

** A copy of your Pharmacy D certificate must be submitted during the application process.**

Candidates interested in positions for this classification must possess a current license as a Registered  Clinical Pharmacist PharmD (PHD) from the Maryland State Board of Pharmacy, 4201 Patterson Avenue, Baltimore, Maryland 21215.

  • Employee in this classification are subject to substance abuse testing in accordance with the Code of Maryland Regulations 17.04.09, Testing for Illegal Use of Drugs.
