BASIC PURPOSE OF POSITION: This position is responsible for performing† routine Clinical Pharmacy Reviews as requested by DDSD case managers based on the criteria specified in the Health and Wellness Policy for home and community based waiver recipients.†
TYPICAL FUNCTIONS:† Performs routine Clinical Pharmacy Reviews as requested by DDSD case managers based on the criteria specified in the Health and Wellness Policy for home and community based waiver recipients.† These reviews provide recommendations to physicians and the team regarding drug interactions, medication dosing, medication contraindications and duplications, drug-disease interactions, necessary medication monitoring, and other health related issues.† Also answers urgent requests regarding specific service recipients (which cannot wait for a formal written response) regarding complex medication issues and complex medical situations.† Provides support to staff within the division, answering questions from those within the agency including case managers, nurses, and other staff regarding health and medication related issues.†
KNOWLEDGE, SKILLS, AND ABILITIES (KSA's): Knowledge of:† Prescription drugs and other medications;† Classification of drugs according to their action, use and dosage;†Pharmaceutical calculations used in weighting and measuring drugs.† Ability to: Review and interpret prescriptions; Review and interpret drug therapy regimens for overall appropriateness; Effectively communicate recommendations, both orally and in written form, to physicians and team members; †Analyze the effects of various combinations of drugs; and, Evaluate a situation accurately and implement an effective course of action.
MINIMUM QUALIFICATIONS: Valid license to practice pharmacy in the state of Oklahoma plus 1 year of experience as a pharmacist.† Clinical experience, particularly in the area of developmental disabilities and/or psychotropic medications, is preferred.†
LICENSE/CERTIFICATE: Valid license to practice pharmacy in the state of Oklahoma is required.† Licensure as a preceptor is preferred.
NOTES: A conditional offer of employment to final candidate will be contingent upon a favorable background check and a substance abuse screening.
