Main Purpose of Job

The main purpose of this position is to provide health care and disease prevention information to medically underserved populations under the direction of the Asthma Community Health Nurse or other professional staff within the Chronic Disease Prevention and Control Division of the St. Mary's County Health Department. This position schedules dates, times and locations for asthma health education and promotion sessions, assists in preparing and assembling materials for asthma health education and promotion sessions, and explains and reinforces the health care services and resources available. The Community Health Outreach Worker will also interact with private and public entities to identify target populations for referral, assist with administrative and outreach support as needed for the department, and perform a range of duties with direction from the immediate supervisor. This position will have regular and recurring interaction with diverse populations and will be responsible for the outreach and awareness initiatives as related to, but not limited to:  Asthma health/promotion, Cancer Control programs, Tobacco Use Prevention and Enforcement, Cardiovascular Health Programs, School and Child Care Center Wellness Program, etc.

MINIMUM QUALIFICATIONS

Education:    Graduation from an accredited high school or possession of a high school equivalency certificate.

Experience:  One year of experience in providing information on health care and disease prevention within the community.

NOTE:

1.  Experience providing assistance to individuals in a health care or social services setting may be substituted for the required education on a year-for-year basis.

2.  Candidates may substitute U.S. Armed Forces military service experience as a non-commissioned officer in Health Services classifications or Health Services and specialty codes in the health related field of work on a year-for-year basis for the required experience. 

DESIRED OR PREFERRED QUALIFICATIONS

The ideal candidate must have experience providing direct support to medically underserved populations in the community and assisting them in adopting healthy behaviors.

LICENSES, REGISTRATIONS AND CERTIFICATIONS

Employees in this classification may be assigned duties which require the operation of a motor vehicle. Employees assigned such duties will be required to possess a motor vehicle operator's license valid in the State of Maryland.

BENEFITS

Contractual employees who work for an agency covered under the State Employee and Retiree Health and Welfare Benefits Program (the Program), have a current employment contract and work 30 or more hours a week (or on average 130 hours per month) may be eligible for subsidized health benefits coverage for themselves and their dependents.  As a contractual employee, you will be responsible for paying 25% of the premiums for your medical and prescription coverage, including any eligible dependents you have enrolled. The State of Maryland will subsidize the remaining 75% of the cost for these benefits. You can also elect to enroll in dental coverage, accidental death and dismemberment insurance, and life insurance, but will be responsible to pay the full premium for these benefits.
