Main Purpose of Job

The main purpose of this position is to coordinate services for individuals with developmental disabilities or individuals with a mental health diagnosis, and their families.  Specifically, this position will: evaluate individuals' needs and outcome based performance of measures by attending team meetings, creating progress notes, exchanging information with other disciplines, and observing individual interactions; analyze individuals' records to complete annual reviews of social history, including background, family, Day Program relationships, programming, sources of funding, guardianship, burial plan and social service recommendations; maintain a social work record, including social work evaluations, record of correspondence and family contacts, financial papers and other pertinent information; function as an advocate for individuals to assure human and civil rights are respected; coordinate admission hearings for newly-admitted individuals; coordinate individuals' discharge; plan documentation of social services to meet or exceed regulations set forth by HCFA, Title XIX, OHCQ, CMS and Accreditation Council; provide crisis intervention; and, provide short term counseling to individuals when needed for conflict resolution.  This position will also act as a liaison between individuals and their families; work with a Financial Agent in areas such as appointment of appropriate representative payee, changes in sources of funding, transferring funding to agencies when individuals move; and, act as a member of an Interdisciplinary Team.

MINIMUM QUALIFICATIONS

Education: Possession of a bachelor’s degree from an accredited college or university in nursing, social work, psychology, education, counseling or a related field.

Experience: Two (2) years of professional experience in health services, one (1) year of which must have been in the option for which application is made (professional work experience related to support services and programs for individuals with developmental disabilities).

Notes: 1. Applicants may substitute a master’s degree from an accredited college or university in a health or human service field for one year of the required general experience.

2. Experience in the option must be as follows: professional work related to the support services and programs for developmentally disabled clients.

3. A bachelor’s degree from an accredited college or university in another field plus one additional year of professional experience in health services may be substituted for the specific degree.

4. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.

The desirable candidate will have professional experience:

-- working with individuals with developmental disabilities

-- working with individuals with a mental health diagnosis

BARGAINING UNIT STATUS

The incumbent in this position is a member of a covered bargaining unit and will be required to pay a bi-weekly service fee to the exclusive representative of the bargaining unit.
